Well-Being Program Overview

Throughout the year we hold community screening events for our Well-Being Program. Community members can sign up on our waiting list to be contacted for our next screening. 

Our Well-Being Program includes a health risk assessment (HRA), screenings with instant results, and coaching to provide the support that you need to improve your well-being. Health screenings are an important tool to assess your risk for chronic diseases such as diabetes, heart disease, osteoporosis, and more.

Health Screenings

Stoughton Health offers a variety of screenings to help you make more informed decisions about your health and wellness. All screenings include a discussion and consultation on how you can improve your results. The following screenings are offered at the Community Health and Wellness Center located at
3162 County Road B in Stoughton.

Breast Cancer Risk Assessment

Stoughton Health’s Breast Cancer Risk Assessment can help you understand your personal breast cancer risk and strategies to reduce your risk. The screening is a free service we offer to help women take charge of their health. Our Breast Cancer Risk Assessment is recommended for all women over the age of twenty-five.

When you schedule your free assessment at Stoughton Health, you will meet with our Breast Cancer Risk Assessment Screener in-person or on the phone. She will ask you questions about your medical and family history. Our medical screening tool will assess your breast cancer risk and you will receive the results during your assessment.

If you are at high risk for breast cancer, you will be scheduled for a follow-up consultation with Stoughton Health Board Certified General Surgeon Dr. Aaron Schwaab to learn more about your results and how to reduce your risk. Dr. Schwaab will guide you through your options so you can take charge of your health.

Heart Disease Screening

Calcium scoring is a heart disease screening offered by Stoughton Health. It is a useful way to detect signs of heart disease while there’s still time to stop it. This simple screening can detect years of plaque build-up in as little as 15 minutes.

You will be notified of the results within one week and if necessary, your doctor may recommend a plan to help you reduce your risk of experiencing a future heart attack.

Who Benefits Most?
The American College of Cardiology and the American Heart Association recommend heart disease screening for men and women age 40-75. If you have any of the following risk factors—you should consider having a heart calcium scoring:

  • high cholesterol levels
  • family history of heart disease
  • diabetes
  • high blood pressure
  • cigarette smoking
  • overweight or obese
  • physically inactive

Schedule Your Screening
A doctor’s order is required. Please contact your physician’s office and request a screening be ordered for you.

The cost of the screening is $75 and payment is due at your appointment. To schedule, please call Stoughton Health Medical Imaging at (608) 873-2299.
