COVID-19 Vaccination Clinics

How do I sign up?
1. Review information about the Moderna COVID-19 Vaccination. We will be administering the Moderna vaccine.  All recipients should review the Fact Sheet below before making an appointment- We will need your informed consent to give you the vaccine.

NOTE: You must be 18 years or older to receive the Moderna vaccine

Serious adverse events from current COVID vaccines are rare. Look at the V-Safe process available to you at https://vsafe.cdc.gov/ for an easy way to monitor yourself for any adverse effects. If you don’t want to use V-Safe you can report serious adverse events directly on VAERS https://vaers.hhs.gov/reportevent.html

 2. Schedule your vaccination appointment. Appointments are available on a first come, first serve basis based on a limited vaccine supply.  Recipients will need a second dose of the vaccine 28 days after the first dose- this appointment will be scheduled at your first appointment (DO NOT schedule your first dose if you will not be available for your second dose on these dates).

Please postpone scheduling your appointment if:

  1. You are ill.
  2. You are in quarantine due to a COVID-19 exposure.
  3. You have received antibody therapy or convalescent plasma for COVID treatment in the past 90 days.
  4. You have received another vaccine in the past 14 days or plan to receive another vaccine within the next 14 days.

3. Preparing for your vaccination appointment:

  • Arrive for your appointment on time at Stoughton Hospital, 900 Ridge Street, Stoughton, Wisconsin
  • Park in designated area. (Parking map included with your confirmation e-mail).
  • Completed COVID Vaccine Consent, Administration Record, and Screening Form (E-Mailed to you with appointment confirmation).
  • Face masks/coverings are required at all times while within Stoughton Hospital. Please plan to be screened upon entrance to the building.
  • No guests who are not receiving vaccine will be allowed to enter the building with you.
  • Wear something that lets staff access your upper arm/shoulder.
  • You will be required to wait 15-30 minutes following your vaccination for side effect monitoring.
