Eligibility

Stoughton Health is pleased to be able to fund local organizations who meet our funding eligibility.

• Organizations seeking funding must focus on at least one of Stoughton Health’s prioritized health issues. For more information on these health priorities, please click here to see our community involvement page.

• Behavioral Health
• Mental Health
• Substance Abuse
• Chronic Disease
• Injuries

• Stoughton Health cannot donate to specific individuals/families, political campaigns, or for-profit organizations.

• Funds cannot be used to pay for the meals/transportation of event presenters.

• Applications must be submitted a minimum of four to six weeks in advance from event date.

• Stoughton Health will attempt to fulfill as many applicable donation requests as possible, however, cannot guarantee every request will be approved due to a large volume of requests.

• Allow 2-4 weeks from application date for final decision.
